Job summary

Western North Carolina Community Health Services (WNCCHS) seeks a mission-minded Registered Nurse Care Manager at the Minnie Jones Health Center in Asheville to provide care management for Medicare and Medicaid patients.

You will collaborate with medical, behavioral health, pharmacy, dental, and care management teams to develop customized care plans and ensure successful transitions from inpatient or ED to accessible, culturally competent primary care.

Qualifications

• Active NC RN licensure required
• BLS certification required
• Bachelor’s degree in nursing or related field
• Two years RN experience required
• Experience with Medicaid patients preferred
• FQHC/community health experience preferred
• Bilingual in English and Spanish and/or Russian a plus
• HIV care experience a plus

Responsibilities

• Provide care management to patients with Medicare and Medicaid.
• Collaborate with medical, behavioral health, pharmacy, dental, and care management teams to create customized care plans.

Western North Carolina Community Health Services (WNCCHS) is a Federally Qualified Health Center (FQHC) serving underserved communities in Western North Carolina. WNCCHS is seeking a mission-minded Registered Nurse Care Manager to provide care management to patients with Medicare and Medicaid at our Minnie Jones Health Center in Asheville. RN Care Managers collaborate with medical, behavioral health, pharmacy, dental, and care management teams to provide customized care plans to improve health outcomes. The RN Care Manager ensures the successful transition of care from inpatient or the emergency department to culturally competent, comprehensive primary care.

QUALIFICATIONS:

• NC state RN licensure
• BLS certification
• Bachelor’s degree in nursing or related field
• Two years’ RN experience required
• Experience with Medicaid patients
• Experience in community health or outpatient care preferred
• Clinical experience in nursing care of the medically complex patient highly desirable
• Familiarity with role of FQHCs in the community
• Bilingual in English and Spanish and/or Russian is a plus
• HIV care experience a plus

Compensation

• Base Salary: $75,000 to $80,000 (based on 40 hours/week)
