Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current state licensure as a Registered Nurse (RN).
• Basic Life Support (BLS) certification.
• Three to five years of nursing experience.

Responsibilities

• Coordinate care and disease management longitudinally for patients with chronic conditions.
• Triages patient concerns to facilitate timely resolution and provider involvement when necessary.
• Collaborate with multidisciplinary care teams across the continuum of care.
• Provide patient education, coaching, and reassessments to update the care plan.
• Assist in transitions of care and connect patients to community resources.
