Job summary

Covenant Health is seeking an RN Care Coordinator II in a Quality Management role. The position emphasizes integrating evidence-based practice, coordinating staff education, and serving as a clinical resource to the care team.

The RN will promote patient-centered care, develop practice guidelines, and monitor outcomes for designated patient populations. Reports to the Manager of Quality Management at the corporate level.

Qualifications

• Bachelor’s degree in Nursing or related field.
• Four years of acute care nursing experience; minimum three years in area of responsibility.

Responsibilities

• Screen patients using case finding criteria and develop care plans.
• Coordinate documentation with CDI to guide the care team.
• Evaluate patient progress and adjust care management plan as needed.
• Collaborate with providers to design guidelines and care pathways.
• Educate patients on medications, treatment plans, and post-discharge needs.
• Lead multidisciplinary discussions and coordinate resources for care transitions.
