• Bachelor’s degree in Nursing or related field.
• Four (4) years of acute care nursing experience; a minimum of three (3) years of experience in area of assigned responsibility.

Responsibilities

• Utilizes case finding criteria to screen patients and gather information to develop a comprehensive plan for the patient.
• Facilitates timely documentation review with the CDI specialist to ensure appropriate documentation guides the care team.
• Uses the care coordination process to evaluate patient progress and adjust the care plan as needed.
• Collaborates with providers and health care team to plan and implement care in a timely manner.
