Southwest Care Center is seeking a full-time RN Care Coordinator to support our Santa Fe clinics and reside in Albuquerque or Santa Fe, NM. This role will be integral to coordinating care for high-risk patients within an interdisciplinary team.

The ideal candidate holds an associate degree in nursing, an active NM or compact-state RN license, and preferably a Bachelor's in Nursing. The position offers a comprehensive benefits package and professional development opportunities.

Qualificações

• Minimum 1 year of RN experience within an ambulatory/primary care environment preferred. LPN or Medical Assisting experience may be considered in lieu of this requirement.
• Completion of an associate degree in nursing required.
• Valid Registered Nurse License in the state of New Mexico or other compact state.

Responsabilidades

• Identifies high risk, high acuity patients that would benefit from care coordination.
• Develops interdisciplinary care plans for identified patients with providers and team members.
• Educates patients and families by explaining the care coordination process and initiating their care plan.
• Coordinates care information with other care providers, resolves issues affecting progression, and fosters peer support.
• Evaluates adherence to and effectiveness of the care plan and modifies as necessary to ensure the best outcome for the patient.
• Documents appropriately in the EMR and supports care protocol improvements.
