Job summary

Geisinger is seeking an experienced registered nurse to assess, triage, and coordinate care for clients within the Healthy Beginnings program. You will support physicians and families, ensuring timely referrals, education, and comprehensive documentation across visits.

Qualifications

• Minimum of 1 year RN work experience is required.
• Leadership experience is preferred but not required.
• Graduate from Specialty Training Program-Nursing (Required).

Responsibilities

• Perform client assessment and triage, coordinate referrals to in-house clinical and community services.
• Provide comprehensive nursing care and support to patients and families.
• Follow up on diagnostic testing to ensure completion and accurate medical records.
• Assist physicians and physician extenders in patient care and documentation.
• Deliver prenatal education and care coordination for clients in the program.
• Maintain up-to-date problem lists and care records for each client.
