Job summary

Scenic Rivers Health Services seeks a full-time RN Care Coordinator for the Tower Medical clinic. You will provide patient-centered care coordination across ambulatory clinics, collaborating with providers and care teams to manage plan of care and follow-up.

Qualifications

• Advanced clinical nursing skills to use discretion and independence while delivering care.
• Maintain a problem-solving mindset and support a culture of safety and accountability.
• Meet defined quality measures and documentation requirements.
• Achieve timely follow-up expectations (ER visits, TOC, results, MyChart).
• Three years clinical experience with chronic disease or case management; five+ preferred.
• EMR experience, EPIC preferred.
• Patient care focus demonstrated in practice.
• Team-centered approach in collaboration with providers and staff.
• Strong organizational and multi-tasking abilities.
• Active MN RN license with required degree.

Responsibilities

• Serve as the contact point, patient advocate, and informational resource for the patient’s care team.
• Assess patient needs, plan care, implement interventions, monitor progress, and evaluate outcomes.
• Identify barriers, social needs, and care gaps; create individualized care plans.
• Provide patient education, next steps, and safety-net instructions.
• Conduct ER and hospital discharge follow-ups; triage over the phone and in person.
• Ensure closed-loop follow-up for labs, referrals, and imaging; Coordinate care
• Collaborate with Providers, RNs, Nurse Manager, and care teams to support co-management.
• Administer tasks per policy and document diligently; maintain MyChart communication.
