We are currently seeking a full-time RN Care Coordinator to join our team at our Tower Medical clinic. This position is responsible for providing patient-centered, relationship-based care coordination across ambulatory clinics and Health Care Home programs. This position will frequently collaborate with Providers, RNs, Nurse Manager and other nursing personnel. The RN Care Coordinator nurse will operate at the top of their RN license by utilizing advanced clinical nursing skills with discretion and independence.
