Job summary

MH-Memphis Hospitals seeks a Registered Nurse to coordinate patient care across the continuum in our clinical/office settings. You will assess physical, psychological, and social needs, educate patients and families, and collaborate with providers to ensure high-quality, compassionate care.

As a trusted care coordinator, you will maintain clear documentation, participate in education and preventive follow-up, and support our MLH mission and values in a fast-paced health system focused on service

Qualifications

• RN license and Associates Degree in Nursing required.
• Registered Nurse in Tennessee or eligible to practice.
• Strong communication and patient-care skills.

Responsibilities

• Assesses the physical, psychological, and social needs of the patient, and improves the care experience for the patient and their family.
• Creates a seamless flow through the health system to facilitate the patient's care.
• Develops and coordinates education, resources, and activities for patients and families.
• Maintains reliable systems to document, track and monitor patient data.
• Performs clinical procedures within the scope of nursing practice and clinical training.
• May perform telephone duties within the scope of RN role.
• Performs other job functions as assigned or requested.
