Job summary

ComForCare Home Care - Oklahoma City is seeking a Registered Nurse to leverage clinical knowledge without direct patient care, focusing on assessment, communication, and care planning during in-home visits.

The RN will travel to clients' homes, perform verbal and observational assessments, review health histories, and develop practical care plans for non-medical caregivers to follow. This role centers on conversation and coordination, not bedside procedures.

Responsibilities

• Travel to clients' homes to complete scheduled, non-clinical nursing assessments.
• Conduct verbal and observational assessments with clients and family members.
• Review medical history, diagnoses, medications, functional abilities, and daily routines.
• Identify risks, support needs, and appropriate non-medical services.
• Develop and update individualized care plans for in-home caregivers to follow.
• Serve as a clinical resource to the care team regarding the client's plan of care.
• Maintain accurate and timely documentation of assessments and care plans.
