Job summary

MASC Medical Recruitment Firm seeks a Registered Nurse to support care coordination, patient engagement, and clinical oversight for patients receiving enhanced care management and wraparound support services in Inland Empire, California.

This hybrid, full‑time position requires in‑person visits and offers a competitive compensation package with benefits, 401(k), and paid time away. The RN will guide care recommendations and coordinate with multidisciplinary teams to improve patient outcomes.

Qualifications

• Active California RN license is required.
• Experience in care coordination or enhanced care management preferred.
• Strong clinical care planning and transitions understanding.
• Knowledge of social determinants of health and community resources.
• Ability to work with patients with complex medical, behavioral, and social needs.

Responsibilities

• Serve as a clinical resource for care coordination and enhanced care management.
• Provide care recommendations within RN scope with physician oversight.
• Support assessments contributing to individualized care plans.
• Promote patient engagement and adherence to care plans.
• Conduct telephonic outreach and patient visits as needed.
• Support transitions of care including post‑discharge follow‑up for high‑risk patients.
• Coordinate services across primary/specialty care, labs, equipment, behavioral health, and community resources.
• Assess barriers to care and coordinate appropriate interventions.
• Participate in multidisciplinary team meetings and care model improvements.
