Job summary

Zohorecruit is hiring a Registered Nurse to support care coordination, patient engagement, and clinical oversight for patients in enhanced care management. The role focuses on guiding care decisions, coordinating transitions, and collaborating with multidisciplinary teams to connect patients with clinical and social resources.

The position offers a hybrid schedule in Riverside, CA, with in-person visits as required.

Qualifications

• RN license active in California
• Experience in care coordination, population health, or enhanced care management preferred
• Strong skills in care planning, patient engagement, and care transitions
• Knowledge of social determinants of health and community resource navigation

Responsibilities

• Serve as a clinical resource for care coordination and enhanced care management programs
• Provide care recommendations within RN scope and physician oversight
• Support assessments contributing to individualized care plans
• Promote patient engagement and adherence to care plans
• Conduct telephonic outreach and patient visits as needed
• Support transitions of care including post-discharge follow-up
• Coordinate services across primary, specialty, labs, equipment, and community resources
• Identify environmental or social barriers to care and coordinate interventions
• Participate in multidisciplinary team meetings and model improvements
• Ensure HIPAA compliance and proper documentation
