Job summary

Qualifications

• Active California RN license.
• Experience in care coordination or enhanced care management preferred.
• Strong clinical judgment and communication skills.
• Knowledge of social determinants of health preferred.
• Able to work in a hybrid role with in-person visits.

Responsibilities

• Serve as clinical resource for care coordination programs.
• Provide care recommendations within RN scope under physician oversight.
• Support assessments for individualized care plans.
• Promote patient engagement and adherence to care plans.
• Conduct telephonic outreach and patient visits as needed.
• Coordinate services across primary, specialty, labs, durable medical equipment, behavioral health and community resources.
• Identify barriers to care and coordinate appropriate interventions.
• Participate in multidisciplinary team meetings.
• Ensure HIPAA compliance and proper documentation.
