Zohorecruit is seeking a Registered Nurse in Riverside, California for a hybrid role focused on care coordination, patient engagement, and clinical oversight for patients with complex medical, behavioral, and social needs.

The RN will guide care recommendations, support transitions of care, and collaborate with multidisciplinary teams to connect patients with clinical and community resources. Compensation is $70,000–$100,000 annually with full benefits.

Qualificações

• Active Registered Nurse license in California and eligibility to work in the United States.
• Experience in care coordination, case management, population health, or enhanced care management.
• Strong communication, documentation, and organizational skills.

Responsabilidades

• Serve as a clinical resource for care coordination and enhanced care management programs.
• Provide care recommendations within RN scope and physician oversight.
• Support assessments for individualized care plans and promote patient engagement and adherence.
• Coordinate services across primary, specialty, labs, DME, behavioral health, and community resources.
• Assist with transitions of care, including post-discharge follow-up for high-risk patients.
• Maintain HIPAA compliance and complete chart audits as required.
