Job summary

VSHP Volunteer State Health Plan, Inc. is seeking a Care Coordinator to visit members at home or in community settings, coordinating health and support services to improve quality of life. You’ll partner with families and a multidisciplinary team to address health, behavioral health, and social needs.

The role requires clinical experience and an active RN license in TN (or compact state) or a licensed MSW. This is an on-site position based in Tennessee with potential for a compressed week after

Qualifications

• 2 years clinical experience required
• Active TN RN license or compact state license; or active MSW license
• Exceptional customer service skills and ability to work with diverse populations
• Must reside in TN, Rhea County
• 8:00am–5:00pm EST schedule with potential compressed week after 1 year

Responsibilities

• Partner with members and families to identify needs and services
• Collaborate with clinical and social support teams to meet health goals
• Conduct face-to-face visits to assess status across domains
• Authorize appropriate home and community-based services
• Maintain compliant documentation and scheduling
• Operate in line with program policies and safety requirements
