Job summary

VSHP Volunteer State Health Plan, Inc is seeking a compassionate CHOICES Care Coordinator to visit members at home or in the community, coordinating services and supporting personal health goals.

This role requires an active RN license in Tennessee or a compact state (or an unrestricted SW license), plus at least 2 years of clinical experience. Ability to provide excellent customer service and telework readiness are essential.

Qualifications

• 2 years - Clinical experience required
• Active RN license in TN or compact state, or unrestricted SW license (LCSW/LMSW/LAPSW)
• Excellent customer service and communication skills

Responsibilities

• Partnering with members and families to identify supports and direct services to meet health goals.
• Collaborates with clinical and social support colleagues to meet health and long-term needs.
• Conduct thorough face-to-face visits to assess current status and needs across physical, behavioral, functional, psychosocial, financial and living aspects.
• Authorize appropriate home and community-based services and ensure needs are met.
