Job summary

VSHP Volunteer State Health Plan, Inc is seeking a compassionate CHOICES Care Coordinator to support members in achieving health and independence through home visits and community-based services.

You will partner with families, coordinate clinical and social supports, assess needs during visits, and ensure services are appropriate and delivered. A TN RN license or valid social work license is required, with a 8:00am-5:00pm EST schedule.

Qualifications

• 3 years of experience in a clinical setting.
• RN with TN license or compact-state license; or MSW with unrestricted license (LCSW, LMSW, or LAPSW).
• Must live within Davidson County, TN.
• 8:00am - 5:00pm EST schedule; compressed week after 1 year may be possible.

Responsibilities

• Partner with members and families to identify needed supports and direct services to meet personal goals for good health, employment and independent living.
• Collaborates with a team of clinical and social support colleagues to meet physical, behavioral health and long-term service needs of each member.
• Conduct thorough face-to-face visits and assess member situations to determine current status and needs.
• Utilize criteria for authorizing appropriate home and community-based services and confirm services are provided.
• Valid Driver's License.
• TB Skin Test for field coordinators.
• Position requires 24 months in role before posting to other positions.
• Immunizations or related medical tests may be required.
• Digital literacy assessment.
