Access Senior Home Care, Inc. is seeking a part-time RN Care Coordinator for field-based duties in the Greensboro/Triad region of North Carolina.

You will conduct in-home assessments, develop individualized care plans, supervise CNAs, and act as a clinical resource during active care. This role requires an active North Carolina RN license, experience in home care or geriatrics, and the ability to travel within roughly a 90-minute radius.

Responsibilities

• Conduct in-home assessments for new and prospective clients.

• Provide CNA orientation, skills validation, and clinical support.
• Perform supervisory visits (at least every 90 days) per NC regulations.
• Maintain compliant clinical documentation.
• Collaborate with leadership on care quality and consistency.

We support aging at home through high-quality caregivers, thoughtful care planning, and coordinated support for families.We are actively serving clients across the Greensboro and Triad regions and are expanding our RN team to maintain strong clinical oversight and a high-quality client experience.The RoleThis is a part-time, field-based RN role focused on both clinical oversight and client-facing care coordination.As an RN Care Coordinator, you’ll conduct in-home assessments, develop care plans, supervise CNAs, and serve as a clinical resource during active care. You’ll also play an important role in helping prospective clients and families understand their care needs and next steps.This role is well-suited for an RN who is comfortable working independently, building relationships, and supporting both caregivers and families in real time.Service Area & TravelThis role supports clients across both our Greensboro service areas, with travel within approximately a 90-minute radius of each location.Client visits are scheduled strategically to group locations when possible.
