Base pay range

$83,000.00/yr - $86,000.00/yr

Job Summary:

The RN Care Coordinator will play a key role in the execution of the Provider Partners Health Plan (PPHP) Model of Care (MOC) through assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet the health needs of PPHP members. The Nurse Care Coordinator will promote quality and cost-effective outcomes, including care coordination for complex and multiple chronic conditions of long-term care residents.

Key Responsibilities:
  • Case identification and risk stratification to assess the needs of PPHP members.
  • Collaborate with the Transitional Care Team to reduce re-admissions and hospitalizations.
  • Conduct and document comprehensive assessments of barriers to care (medical, social, psychosocial, literacy).
  • Coordinate development of integrated care plans in collaboration with nursing staff, leadership, and the interdisciplinary team according to the PPHP MOC.
  • Facilitate appropriate referrals and consultations.
  • Document care plans in the gEHRimed EMR system.
  • Manage outcomes and evaluate achievement of care plan goals.
  • Ensure initial and annual Model of Care training for employees, providers, and contracted personnel associated with the ISNP program.
Qualifications:
  • Current unrestricted RN license in practicing state.
  • Minimum of 3 years clinical care experience.
  • Preferred experience in Skilled Nursing Facility/Long Term Care and case management.
  • Strong nursing expertise in managing complex cases with minimal supervision.
  • Knowledge of Medicare/Medicaid standards and practices.
  • Ability to communicate professionally with interdisciplinary teams.
  • Excellent critical thinking, problem-solving, interpersonal, organizational, multitasking, and teamwork skills.
  • Proficiency in nursing/medical databases and Microsoft Office programs.
  • Positive attitude and reliable transportation (valid driver’s license and insurance).

This position is in-person, Monday through Friday.
