Base pay range

$83,000.00/yr - $86,000.00/yr

Job Summary:

The RN Care Coordinator plays a key role in executing the Provider Partners Health Plan (PPHP) Model of Care (MOC) through assessment, planning, facilitation, care coordination, evaluation, and advocacy to meet the health needs of PPHP members. The role involves promoting quality and cost-effective outcomes, especially for residents with complex and multiple chronic conditions.

Key Responsibilities:
  • Identify cases and stratify risks to establish member needs.
  • Collaborate with the Transitional Care Team to reduce re-admissions and avoid hospitalizations.
  • Conduct comprehensive assessments of barriers to care, including medical, social, psychosocial, and literacy issues, to develop holistic care plans.
  • Coordinate development of integrated care plans with nursing staff, leadership, and interdisciplinary teams.
  • Facilitate referrals and consultations as appropriate.
  • Document care plans in the gEHRimed EMR system.
  • Manage and evaluate outcomes against established goals.
  • Ensure initial and annual Model of Care training for employees, providers, and contracted personnel involved with the ISNP program.
Qualifications:
  • Current unrestricted RN license for practicing state.
  • Minimum of 3 years clinical care experience.
  • Preferred experience in Skilled Nursing Facility/Long Term Care and case management.
  • Strong expertise in managing complex cases with minimal supervision.
  • Knowledge of current medical practices and Medicare/Medicaid regulations.
  • Ability to communicate professionally with interdisciplinary teams.
  • Excellent critical thinking, problem-solving, organizational, and communication skills.
  • Proficiency in nursing/medical databases and Microsoft Office.
  • Positive attitude, valid driver’s license, and reliable transportation.

Work schedule: Monday through Friday, in person.
