Job summary

The Ohio State University Physicians, Inc. is seeking a Clinical Nurse Care Coordinator (RN) in Columbus, OH to collaborate with clinic teams and provide evidence-based, patient-centered care tailored to high-risk patients.

You will focus on medically complex patients, disease registries, and care transitions, while coordinating with families and other agencies to ensure comprehensive, continuous care and improved health outcomes.

Qualifications

• Associate Degree in Nursing from an accredited program and Ohio RN licensure.
• Current CPR/BLS certification required for patient care.
• Experience with triage and clinic/urgent care settings preferred.
• Ability to educate patients and families and communicate clearly.

Responsibilities

• Collaborate with clinic team members to provide evidence-based, patient-centered care.
• Focus on high-risk medically complex patients and care transitions.
• Serve as liaison with families and other agencies to improve communication.
