Job summary

Life Care Center of Altamonte Springs is seeking an experienced Registered Nurse Care Coordinator to support its ISNP program. This temporary assignment covers an FMLA leave and involves coordinating care with clinicians, facility staff, residents, and interdisciplinary teams to ensure high-quality patient care.

The RN will maintain documentation, support quality initiatives, assist with clinical coordination for geriatric patients, participate in meetings, and help with education for staff.

Qualifications

• Active, unrestricted RN license in Florida or eligibility before start
• ADN or higher completed
• Current BLS certification
• Ability to lift up to 30 pounds
• Reliable transportation and local facility travel
• Fully vaccinated for COVID-19 per requirements
• Pre-employment drug screen required

Responsibilities

• Ensure compliant clinical documentation.
• Collaborate with interdisciplinary care teams for patient-centered care.
• Assist providers and care teams with care coordination.
• Support Medicare Part A/B/D benefit determinations.
• Coordinate therapies and follow-up care.
• Participate in care planning and family conferences.
• Support quality initiatives and documentation reviews.
• Educate facility staff through in-services.
• Communicate verbal orders per state regulations.
• Coordinate diagnostics and interventions for acute conditions.
