Job summary

Iredell Health System is seeking a dedicated Care Coordinator to oversee patient care coordination across disciplines and facilitate progression and discharge planning. You will work closely with medical, nursing and ancillary teams to optimize resource use and patient outcomes.

The role requires active RN license in North Carolina, BLS certification, and at least 2 years of hospital experience, with preferred experience in utilization and discharge planning.

Qualifications

• Currently licensed as a Registered Nurse by the North Carolina Board of Nursing.
• BLS Certification through American Red Cross or American Heart Association.
• Minimum of 2 years of clinical experience in a hospital setting.
• Previous utilization and/or discharge planning experience preferred.
• Excellent organizational and communication skills required.

Job description

The Care Coordinator is responsible for the coordination of patient care through multidisciplinary collaboration with medical, nursing and ancillary departments. The Care Coordinator assists with the planning of patient care including, but not limited to, facilitation of patient progression and discharge planning. The Care Coordinator is responsible for assuring effective use of available hospital and community resources and monitoring patient outcomes through concurrent utilization review and proactive discharge planning. Has full access to patient health information.

Shift: M-F, 8:00 AM - 4:30 PM, weekend and holiday rotation

Requirements

Essential Physical Requirements

• Must be able to possess full range of body motion to pass a basic FIT test to include bending, stooping, standing and sitting for extended periods of time.
• Must be able to lift 30 pounds.
