Job summary

Iredell Memorial Hospital Incorporated in Statesville, NC seeks a Care Coordinator to align patient care across medical, nursing and ancillary teams. You will support care planning, progression, and discharge planning to ensure smooth transitions.

The role requires strong organizational and communication skills, NC RN license, BLS, and 2+ years hospital experience. BSN preferred and use of hospital/community resources for patient outcomes is essential.

Qualifications

• Currently licensed as a Registered Nurse by the North Carolina Board of Nursing. BSN preferred.
• BLS Certification through American Red Cross or American Heart Association
• Minimum of 2 years of clinical experience in a hospital setting.
• Previous utilization and/or discharge planning experience preferred.
• Excellent organizational and communication skills required.

Description

The Care Coordinator is responsible for the coordination of patient care through multidisciplinary collaboration with medical, nursing and ancillary departments. The Care Coordinator assists with the planning of patient care including, but not limited to, facilitation of patient progression and discharge planning. The Care Coordinator is responsible for assuring effective use of available hospital and community resources and monitoring patient outcomes through concurrent utilization review and proactive discharge planning. Has full access to patient health information.

Shift: M-F, 8:00 AM - 4:30 PM, weekend and holiday rotation

Requirements

Essential Physical Requirements

• Must be able to possess full range of body motion to pass a basic FIT test to include bending, stooping, standing and sitting for extended periods of time.
