University of Virginia invites a Registered Nurse Care Coordinator to join the UMA Clinic in Charlottesville. This role focuses on Transitional Care Management and Chronic Care Management for an internal medicine population, with autonomy and a supportive team.

You will coordinate care, use nursing judgment, and help expand CCM program, with flexible scheduling (five 8-hour days or four 9-hour days plus one 4-hour day) and potential remote days after orientation.

Qualifications

• RN with experience in primary care coordination.
• Experience in Transitional and Chronic Care Management preferred.
• Licensed to practice as a Registered Nurse in Virginia.

Responsibilities

• Manage and support Transitional Care Management and Chronic Care Management activities.
• Coordinate care for patients with ongoing and evolving healthcare needs.
• Use nursing judgment to identify priorities, support continuity, and engage patients in their care.
• Collaborate within an academic internal medicine environment including nurses, physicians, residents, and the care team.
• Contribute to the expansion of the Chronic Care Management program.
