Job summary

The Rector & Visitors of the University of Virginia is seeking a Registered Nurse Care Coordinator (RNCC) to organize patient and family-centered care activities across the care continuum. The RNCC will develop and implement individualized care plans, coordinate resources, and support transitions of care with a focus on quality outcomes.

Required BSN, Virginia RN license, and BLS certification; 1 year RN experience with 3 years preferred.

Qualifications

• Education: Bachelor of Science in Nursing from an accredited nursing program.
• Experience: 1 year as RN required; 3 years related RN experience strongly preferred.
• Licensure & Certification: Licensed to practice as a registered nurse in the Commonwealth of Virginia; AHA BLS required; specialty board cert preferred.

Responsibilities

• Coordinate patient care activities across inter-professional teams to ensure safety and continuity of care.
• Provide patient/family education and engage in shared decision‑making.
• Support medication management and care transitions across locations.
