Job description

The Care Coordinator RN (CC RN) is responsible for assessing patients' transitional care needs, coordinating care across the continuum, and engaging with patients and families to ensure care goals are met.

Key Duties Include

• Conducting psychosocial and functional assessments, developing individualized discharge plans, and facilitating patient/family education.
• Collaborating with physicians, interdisciplinary teams, and utilization review nurses to evaluate medical, psychosocial, and social determinants of health needs.
• Managing discharge planning by identifying barriers, ensuring timely resource allocation, and facilitating smooth transitions to appropriate care settings.
• Participating in interdisciplinary rounds, monitoring care progression, and resolving delays to promote efficient patient discharge.
• Documenting assessments, interactions, and care plans accurately and maintaining compliance with organizational policies.
• Supporting professional development, serving as a mentor, and assisting in departmental goals.

Qualifications Include

• Associate or Bachelor's Nursing degree.
• RN license (single state or multi-state compact) and BLS certification.
• Minimum of 1 year of nursing experience in an acute care setting.
