Job summary

The Clinical Nurse Care Coordinator at The Ohio State University Physicians, Inc. delivers evidence-based, patient-centered care with a focus on high-risk and complex cases. You will coordinate plans, track care transitions, and support families while collaborating with the clinic team.

As an Ohio RN, CPR/BLS certified professional, you will promote safety, education, and communication across care settings, helping patients navigate their care journeys efficiently.

Qualifications

• Requires an Associate Degree in Nursing from an accredited program.
• Active Ohio RN license and CPR/BLS certification.
• Experience with triage and clinical/urgent care settings.
• Ability to maintain safety, quality, and infection control standards.
• Ability to educate patients and families on disease and care.
• Strong written and verbal communication and interpersonal skills.
• Demonstrated poise, tact and diplomacy; analytic problem-solving.

Responsibilities

• Collaborate with clinic team to deliver evidence-based, patient-centered care.
• Focus on high-risk medically complex patients and care transitions.
• Serve as liaison with families and other agencies to improve communication.
• Keep current on standards and assist with programs and processes.
