Qualifications

• Basic understanding of Medicare, Medicaid, and Managed Care regulations.

Responsibilities

• Care Coordination is a collaborative process to assess, plan, implement, coordinate and evaluate options to meet patient health needs.
• Facilitate progression of care and monitor patient progress to ensure plan is patient-focused and cost-effective.
