Job summary

St. Tammany Health System in Covington, LA seeks an experienced Outpatient Care Coordinator RN to assess, plan, implement, coordinate and evaluate care options for patients within the continuum of care.

The role focuses on facilitating progression of care, monitoring progress and ensuring a patient-centered, efficient, evidence-based plan. LA RN licensure is required; BSN preferred and BLS certification is expected.

Qualifications

• Graduate of accredited school of Nursing; BSN preferred.
• Current LA RN licensure.
• Working knowledge of alternative payment methodology, insurance coverage, post-acute providers and community resources.
• Demonstrated ability in planning, organizing, leadership and communication.
• Strong problem-solving skills.
• Current BLS certification through ILCOR courses with manikin check-off.
• Two (2) years of Acute Care Hospital, ED, Case Management or Utilization Management experience preferred.

Responsibilities

• Care Coordination RN assesses, plans, implements, coordinates and evaluates options to meet health needs; ensures high-quality, evidence-based care.
• Outpatient Care Coordinator RN assists in overcoming health system barriers and improves outcomes across the continuum of care.
