St. Tammany Health System in Louisiana is seeking a Care Coordinator to support care planning and patient progress in a hospital setting. The role focuses on coordinating services and evaluating care plans to improve outcomes and efficiency.
Ideal candidates are RNs with current state licensure, 3–5 years of acute care experience, and strong communication and organizational skills. Part-time position with weekday shifts and limited weekend coverage.

Care Coordination is a collaborative process in which the Care Coordinator assesses, plans, implements, coordinates, and evaluates options and services to meet an individual’s health needs. The Care Coordinator facilitates progression of care and monitors the patient's progress to ensure that the plan of care and services provided are patient focused, high quality, evidence based, appropriate to patient needs, efficient, and cost effective. Care Coordination improves access to services to improves outcomes and reduce over-utilization of resources.
Must possess good physical health. Some requirements include but are not limited to standing, sitting or walking for long periods of time. Lifting at least 10 pounds is required. Must be able to work with a moderate level of noise.
