Job summary

UAB Medicine in Alabama seeks a Registered Nurse Care Coordinator (RNCC) to join a Magnet-designated team delivering high-quality care across inpatient and ambulatory settings, including the ED. The RNCC coordinates discharge planning, post-discharge referrals, and care coordination for complex patients within the care continuum.

Requirements include a Bachelor’s degree in Nursing with 3 years of experience or an Associate’s with 5 years, and current RN licensure in Alabama.

Qualifications

• Bachelor's degree in Nursing and three (3) years of related experience required OR Associate's degree in Nursing and five (5) years of related experience required.
• Currently licensed or eligible to be licensed as a Registered Nurse (RN) by the Alabama Board of Nursing.
• Certified in Basic Life Support (BLS) from the American Heart Association is required or should be obtained within 30 days of employment. Depending on the unit, ACLS/PALS may be required.

Responsibilities

• Coordinate, negotiate and obtain services and resources for the care of patients.
• Review case criteria to determine medical necessity for admission and continued stay.
• Provide discharge planning and care coordination that identifies cost-effective methods of patient care.
