Qualifications

• Graduate of an approved school of nursing with current registration in Massachusetts.
• Bachelor of Science Degree in Nursing is required for newly licensed nurses and external candidates.
• 1+ years of acute care experience required
• 1 year inpatient acute case management experience required

Responsibilities

• Care facilitation: develop provisional plan and discharge date with patient/family.
• Review daily treatment plan with physicians, nurses and family to ensure coordination.
• Participate in patient care rounds to adjust plan and monitor progress.
• Collaborate with other departments to expedite interventions and services.
• Discharge planning: assess needs, coordinate meetings, and finalize plans prior to discharge.
• Utilization management: address payer issues and ensure appropriate care and documentation.
