Job summary

Wellstar Health System's Cobb Hospital is seeking a registered nurse Care Coordinator for a full-time day shift onsite. You will assess transitional care needs, coordinate across the continuum, and engage with patients and families to ensure care is met.

You will collaborate with the utilization review nurse, physicians, and the care team to optimize discharge planning and resource use. Strong communication and organizational skills are essential.

Qualifications

• Associate's Degree in Nursing with Georgia RN license.
• RN license (single state or Multi-state Compact) and BLS.
• Minimum 1 year nursing experience in the acute care setting.
• Excellent written and verbal communication.
• Ability to work under stress in a fast-paced environment.
• Knowledge of local, state and federal regulations.
• Ability to collaborate with physicians and care team.

Responsibilities

• Assess patients' transitional care needs and coordinate care.
• Facilitate discharge planning and resource coordination.
• Collaborate with physicians, UM, and care team to ensure progress.
• Document care progress and ensure timely communication.
• Identify barriers to discharge and initiate post-acute referrals.
