Job summary
The Mental Health Association of the Southern Tier is seeking a compassionate Registered Nurse (RN) Care Coordinator for the Second Shift to join our Crisis Stabilization Center. This role supports individuals in crisis within a 24/7 OMH/OASAS facility, providing medical care, assessments, and care coordination alongside a multidisciplinary team.

Qualifications
- Current New York State RN license.
- Associate's degree or higher in Nursing.
- At least two years in mental health and/or substance use services.
- Experience with children, youth, and adults.
Responsibilities
- Conduct nursing assessments, triage, and medical screenings for levels of care.
- Monitor medical status, vital signs, symptoms, and overall health.
- Provide basic nursing care, crisis interventions, psychoeducation, de-escalation.
- Collaborate with psychiatric providers and multidisciplinary team.
- Develop and implement service, safety, and discharge plans.
- Coordinate referrals to mental health, substance use, primary care, and community resources.
- Address barriers to recovery by linking to housing, transportation, and essentials.
- Maintain accurate clinical documentation and ensure OMH/OASAS compliance.
