Job summary

Witham Health Services in Lebanon, IN seeks an RN - Care Coordinator to lead care coordination for patients with chronic and complex needs. The role collaborates with patients, families, physicians, and the care team to implement a shared plan of care.

Schedule: Mon–Fri, 8am–4:30pm. Requirements include an Indiana RN license and 3–5 years in clinical or community health; BSN preferred. Experience with health IT and community resources is preferred.

Qualifications

• Graduate from an accredited school of nursing; BSN preferred.
• Active RN license in Indiana.
• 3–5 years' experience in clinical or community health settings.
• Experience in care coordination/case management preferred.
• Strong leadership, communication, and counseling abilities.
• Proficient with email, mobile devices, and health IT platforms.
• Experience using health IT systems and data reports preferred.

Responsibilities

• Coordinate care across settings to achieve patient/family-centered goals.
• Coach patients and families in self-management of chronic disease.
• Develop and monitor care plans with input from patient, family, and providers.
• Ensure timely follow-up, medication reconciliation, and test result tracking.
• Collaborate with primary care, specialists, and community resources.
