Job summary

Provider Partners Health Plan in Jasper, IN seeks a Nurse Care Coordinator to drive the Model of Care through assessment, planning, and coordination for members with complex needs. You will work with the interdisciplinary team to promote high-quality, cost-effective outcomes in long-term care settings.

The role requires an active RN license, 3+ years of clinical care experience, and proficiency with EMR systems. Travel and driver’s license may be necessary to support the program.

Qualifications

• Current unrestricted RN license for the practicing state.
• Minimum of 3 years clinical care experience.
• SNF/Long Term Care experience preferred.
• Case management experience preferred.
• Strong nursing expertise in managing complex cases.
• Knowledge of Medicare/Medicaid.
• Ability to work with interdisciplinary team.
• Excellent communication, organization, and multitasking.
• Proficiency in nursing databases and Microsoft programs.
• Valid driver’s license and car insurance.

Responsibilities

• Case identification and risk stratification to assess needs.
• Collaborate with Transitional Care Team to reduce readmissions.
• Assess barriers to care and develop the care plan.
• Coordinate integrated care plan with nursing and interdisciplinary team.
• Facilitate referrals and consultations.
• Document care plans in the gEHRimed EMR.
• Manage outcomes and evaluate goals.
• Ensure Model of Care training for staff and contractors.
