Qualifications

• Three to five years of clinical experience, with strong communication and critical thinking abilities.

Responsibilities

• Guide Care Navigators to ensure collaborative care management.
• Outreach to patients with chronic conditions meeting program eligibility.
• Assess patient needs, build rapport, and monitor adherence to care plans.
• Educate patients and families about treatment plans, medications, and services.
• Collaborate with providers to optimize care and close gaps in care.
