Job summary

Wellstar Cobb Hospital is seeking a Care Coordination Daytime Team Lead (RN) for FT Days. You will mentor a care coordination team, perform assessments, coordinate discharge planning, and advocate for patients and families throughout acute and ambulatory care.

The role emphasizes collaboration with the interdisciplinary team, data-driven dashboards, and ongoing process improvement to ensure timely and effective transition of care. Excellent leadership and communication are essential.

Qualifications

• 3+ years of care coordination experience in a hospital setting.
• RN licensure and current/active state license required.
• Strong leadership and coaching abilities for multidisciplinary teams.

Responsibilities

• Lead and mentor the care coordination team to deliver patient-centered care.
• Oversee assessments, discharge planning, and transition of care.
• Collaborate with interdisciplinary teams and manage workflow on the unit.
• Monitor quality indicators and drive improvement initiatives.
• Document care plans and outcomes in EPIC and maintain records.
