Job summary

Wellstar Health System is seeking a Care Coordination Team Lead (RN) for Cobb Hospital. This full-time daytime role focuses on mentoring a care coordination team during acute hospital stays and ambulatory case management, including discharge planning and family education.

You will collaborate with interdisciplinary teams to ensure high-quality patient outcomes and care transitions. Qualifications include a GA RN license, BLS/ACLS, 3+ years in care coordination, and strong communication.

Qualifications

• 3+ years of recent care coordination experience in a hospital setting is required.
• Strong interpersonal and verbal/written communication skills.
• Ability to coach, lead change and guide an interdisciplinary team.

Responsibilities

• Mentor and lead the care coordination team.
• Coordinate discharge planning and transitional care for defined patient populations.
• Collaborate with interdisciplinary teams and ensure quality indicators are met.
• Provide education to patients/families and monitor care outcomes.
• Assist with staffing, scheduling, and onboarding of new hires.
