Qualifikationen

• Experience with discharge planning and care transitions.
• Ability to mentor and lead a care coordination team.

Aufgaben

• Initiates assessments for necessity and appropriateness of health services.
• Assesses insurance and coverage issues; identifies providers.
• Collaborates with interdisciplinary care team for transitional care planning.
• Provides education to patient/family on medical recommendations and care plan.
• Manages discharge planning and post-discharge follow-up.
