Wellstar Cobb Hospital is seeking a Care Coordination Team Lead (RN) to mentor the care coordination team and support leaders in delivering patient-centered care during acute hospitalization and ambulatory case management. The role emphasizes hands-on support and education for staff and patients alike.

The position focuses on discharge planning, transitional care, and collaboration with the interdisciplinary care team to ensure timely, efficient care and optimal patient outcomes.

Qualifications

• Minimum 3 years recent Care Coordination experience in a hospital setting.
• Exposure to multiple medical populations preferred.
• Licenses/Certifications: RN and active licensure required and/or multi-state compact where applicable.

Responsibilities

• Lead and mentor the care coordination team using evidence-based practices.
• Support discharge planning and transitional care across care settings.
• Collaborate with interdisciplinary care team to develop and monitor plans of care.
• Provide education and advocacy to patients and families.
• Monitor quality indicators and participate in performance improvement initiatives.
• Assist with staffing, coverage planning and training new hires.
