Job summary

Marion Health is seeking a Nurse Case Manager to coordinate care across patients, families, physicians, and home health agencies to ensure high-quality, cost-effective health and hospital services. The role requires an active Indiana RN license and strong communication skills.

Ideal candidates will have 3+ years of nursing experience in acute settings and may hold CCMC/ACMA CM certifications. BLS is required or obtained during orientation.

Qualifications

• Bachelor Degree in Nursing from an accredited university OR Associate's Degree in Nursing with 2 years Case Management experience.
• Current Indiana Registered Nurse License.
• Three (3) years of recent nursing experience in an acute hospital or related setting.
• BLS certification or must obtain during Orientation and prior to reporting to department.

Responsibilities

• Coordinates care between patients, families, physicians, home health agencies and other care providers to ensure high-quality cost-effective health and hospital services.
