Job summary

St. Joseph’s Health in New Jersey seeks an experienced Care Management leader to oversee department operations and guide care coordination across the continuum. The role focuses on improving patient outcomes through effective utilization management, discharge planning, and staff mentorship.

The position calls for a Bachelor’s in Nursing or a related field, with a Master's preferred, plus 3–5 years of related experience and NJ licensure as an RN or SW. CCM/ACM certification is favored.

Qualifications

• Requires a Bachelor's Degree in Nursing or closely related field; Master's degree preferred.
• Licensure as a Registered Nurse by the State of New Jersey or as a Licensed Social Worker by the State of New Jersey.
• Case Management Society of America Certification CCM or ACM preferred.

Responsibilities

• Manages the department to ensure Care Management operations and workflow.
• Provides clear communication across the continuum to improve patient outcomes.
• Lends guidance to staff in utilization management, discharge planning and care coordination issues.
• Collaborates with Care Management Leadership and Administration to implement and evaluate an integrated Care Management program.
