Qualifications

• Bachelor's of Nursing upon hire or 1-3 years experience equivalent required.
• Minimum of three (3) years experience in Care Coordination or Case Management.
• Registered Nurse: CA (RN:CA) required.
• Certified Case Manager (CCM) preferred.

Responsibilities

• Oversee and lead the daily operations of the Care Coordination department, ensuring high-quality, patient-centered care transitions.
• Manage and mentor Care Coordination RNs, monitor performance, ensure regulatory compliance, and collaborate with interdisciplinary teams to optimize patient flow and effectiveness.
• Demonstrate strong leadership, strategic planning, and clinical expertise to drive continuous improvement in patient care coordination.
