Qualifications

• Bachelor's Degree in Nursing (BSN) or Master's in Social Work (MSW) or equivalent education and experience.
• Minimum of three (3) years of clinical care coordination experience or three (3) years of acute nursing team lead/supervisory experience.
• Operational experience, implementation, and staff development in healthcare environment.
• Current RN license in California (DDS for MSW not required).
• CCM or ACM certification required within 1 year of hire.
• Apply clinical guidelines to ensure progression of care; communicate effectively; analyze and delegate.

Responsibilities

• Provide operational leadership for Care Coordination through supervision, coaching, and performance management.
• Coordinate and implement Care Coordination programs and workflows to support patient progression and outcomes.
• Manage daily department operations including staffing, scheduling, productivity, and resources.
• Supervise front-line Care Coordination staff ensuring accountability and service excellence.
• Ensure timely completion of patient assessments, discharge planning, and care transition activities per standards.
