Job summary

UofL Health is seeking a Manager, Care Management to lead the care coordination team and optimize clinical resources. You will drive admission assessments, readmission risk reduction, and cross-functional collaboration to improve patient outcomes.

You will develop compliant policies, monitor data for service improvements, and oversee payroll and PTO processes to ensure staff satisfaction and operational efficiency.

Qualifications

• Demonstrated leadership in care coordination or hospital operations.
• Experience guiding multidisciplinary teams to improve outcomes.
• Understanding of regulatory, ethical, legal, and accreditation standards.

Responsibilities

• Conduct admission assessments and readmission risk assessments to determine care level.
• Develop policies to ensure regulatory and accreditation compliance.
• Monitor data collection to improve length of stay, readmissions, and resource use.
• Document multidisciplinary clinical assessments and oversee daily rounds.
• Oversee PTO requests in SmartSquare and staff scheduling per guidelines.
• Lead staff in alignment with hospital philosophy and business principles.
• Process payroll in Kronos bi-monthly to ensure accurate compensation.
• Evaluate technology and tools to improve processes and revenue.
• Collaborate with clinical and financial staff to meet hospital goals.
