UofL Health is seeking a Manager of Care Management to lead the Care Coordination team within its Louisville hospital system. You will drive patient outcomes by optimizing length of stay and readmission metrics, and ensure compliance with regulatory standards and payer requirements.

The role requires RN with a BSN, five years in care management, two years acute hospital experience, and active BLS-CPR certification; Master’s degree preferred.

Qualifications

• RN with a BSN is required; MSN/MBA preferred.
• Minimum 5 years as Care Manager/Utilization Review Nurse.
• 2 years acute hospital experience preferred.
• Unrestricted Kentucky RN license required.
• BLS-CPR certification required; ACM/ CCM certifications preferred.

Responsabilités

• Promotes optimal patient outcomes by managing care coordination and resource use.
• Develops policies to ensure regulatory, ethical, legal, and accreditation standards.
• Monitors length of stay, readmission risk, and care transitions to optimize reimbursement.
• Oversees PTO tracking and payroll processes in department systems.
• Collaborates with clinical and financial staff to meet hospital goals and improve care delivery.
