Job summary

The National CARDIO-VET Network is recruiting for Registered Nurse Health Care Navigator positions at VA NY Harbor Healthcare System in New York, NY. The nurse coordinates triage and directs concierge-level care for Veterans, including arranging transportation and lodging for cardiovascular procedures.

The role reports to the CARDIO-VET Nurse Manager and may travel up to 25% across the CARDIO-VET network; telework is considered per agency guidelines but this is not a fully virtual position.

Responsibilities

• Coordinates transportation and lodging to support Veterans accessing cardiovascular procedures.
• Collaborates with interdisciplinary team to plan, implement and evaluate care.
• Provides patient education, self-management support, and coordinates care across the continuum.
