Job summary

CVS Health is seeking a RN Case Manager for a remote, work-from-home role across the United States, with virtual training. This telephonic position supports fully and/or self-insured clients, applying clinical guidelines to determine benefits and coordinate care.

Qualifications

• RN with 5+ years' experience, including at least 1 year in a hospital
• Active multistate license via Nurse Licensure Compact or state-specific licenses for each state supported
• 1+ years' experience documenting electronically using a keyboard
• 1+ years' experience in Oncology

Responsibilities

• Telephonic case management for fully/self-insured clients
• Apply criteria and guidelines to determine benefits and coordinate care
• Use clinical judgment to reduce risk factors and address barriers
• Assess information from multiple sources to address conditions affecting care
• Escalate needs and present cases in case conferences for multidisciplinary care
